Registration Form and Information Packet
Registration packets are physically mailed to seniors the week of Spring Break. However, the registration form may be available sooner via the PDF fill-able form link below - please download it to your desktop, ensure that it is printed double-sided and that both sides are completed AND SIGNED! Once it's complete, please turn in to the Carson High School Finance office - if attending the Six Flags trip, include the $60 fee.
PLEASE NOTE: We have limited (first-come, first-served) scholarships available for the Six Flags trip. If you are in need, please complete the registration form (ensuring to check the box for scholarship) and turn it in to the CHS Finance office as soon as possible.
